Ultrasound: An Triage Tool in Acute Right Flank Pain With Reference to Appendicitis

【Objective】
Ultrasound with color Doppler study is main objective to rule out common gastrointestinal and urological causes of acute pain, secondly to study morphology and vascular pattern of borderline appendicitis.
【Material & methods】
We studied 350 patients of acute right flank pain suspected appendicitis. Only 84 patients were selected to have appendicitis. Rest 266 patients other than appendicitis were ruled out. Suspected cases of border line appendicitis were studied for morphology and vascular pattern with reference to specificity and sensitivity, which sometimes problematic even on CT.
【Results】
Out of 350 patients only 84 patients were found to have borderline appendicitis on ultrasound by studying different morphological and vascular pattern. Out of Rest 266 patients, 136 patients were normal, while others 130 patients having different cholecystitis, renal and ureteric calculi and some gynaecological problems ruled out.
Borderline thickened appendix showed increased vascularity, as more than 3mm linear and curvilinear Doppler signals. For morphology thickness and distension, more 8 mm and non compression and tenderness at right iliac region. This all confirming the diagnosis
【Conclusion】
Ultrasound study of borderline appendicitis morphology and vascular pattern with reference to specificity and sensitivity are helpful in suspected borderline appendicitis. Further may avoid unnecessary CT radiation. So ultrasound may become an triage tool in acute right flank pain and suspected appendicitis.
