A Case of Hepatic Focal Nodular Hyperplasia (FNH): Usefulness of Three-Dimensional (3-D) Color Doppler Ultrasonographic Imaging

We encountered a case of focal nodular hyperplasia (FNH) in an 18-year-old woman. Ultrasound examination showed an isoechoic tumor 6cm in diameter in the right lobe of the liver, and a central hypoechoic lesion with an acoustic shadow was visualized at the center of the tumor. The vasculature of the tumor was scanned by 2-D color Doppler and 3-D velocity imaging. Color Doppler images obtained using the velocity and power modes demonstrated a hypervascular tumor with enlarged afferent blood vessels and several centrifugal flows from the center of the tumor. Fast Fourier transformation (FFT) demonstrated arterial pulse wave in the tumor vessels. We were able to evaluate intratumoral flow characteristics more precisely with 3-D velocity images. Results of 3-D imaging corresponded to those of hepatic angiography when tumor vessels were visualized in the early arterial phase. We believe that 3-D color Doppler US imaging will become an established modality for use in US flow imaging.
