We reviewed 91 patients with renal cancer to determine whether tumor thrombus in the renal vein or inferior vena cava is better diagnosed by ultrasonography, CT scan or angiography. Ultrasonography provided a diagnostic accuracy of 90% in the absence of tumor thrombus, 8% in the presence of tumor thrombus in the renal vein and 90% in the presence of intra-caval tumor thrombus. A similar diagnostic accuracy was obtained by CT scan or angiography. Since the presence or absence of intra-caval thrombus most influences the surgical procedure, and since the latest ultrasonic equipment makes it possible to diagnose intra-caval thrombus more correctly, we believe that ultrasonography should be the first choice for patients with renal cancer who need to be examined.
