Two patients with the Björk-Shiley mitral prosthesis showing paravalvular leakage were presented. In those patients, M-mode echocardiography showed hyperkinetic motion of the left ventricle and early diastolic hump of the disk. Using color flow mapping, systolic abnormal flow signal in the vicinity of the suspected dehisence was observed in the left ventricle, while the mitral regurgitant jet in the left atrium could not be visualized in both patients because of artifacts. We concluded that Doppler color flow mapping is useful in detecting paravalvular leakage due to mechanical prosthetic valve dysfunction in the mitral position.
