In order to evaluate regional LV wall function noninvasively, 8 patients with anteroseptal old myocardial infarction (OMI), 7 with hypertension (HTN), 8 with dilated cardiomyopathy (DCM), 7 with hypertrophic cardiomyopathy (HCM), and 10 normals were studied by using color encoded regional wall stress (WS) image on 2-D echocardiography.
Method; Long axis 2-D echocardiography in endosystole was digitized into 512×480 pixels utilizing an image processor. Regional WS was calculated by the Law of Laplace, based on ellipsoidal model.
Circumferential and meridional WS were expressed by color encoded image on 2-D echocardiography.
Systolic pressure was measured as brachial artery pressure during recording echocardiography.
Results
1) Circumferential regional WS were higher in normals than in DCM, HCM, HTN, and anteroseptal MI.
2) WS of HTN showed the same regional distribution as normals, but it was slightly lower than normals.
3) Regional variances of WS were higher in DCM than in normals.
4) WS of HCM, especially on the region of hypertrophic septum, were significantly lower than normals.
5) WS on the infarcted wall is lower and that on the intact posterior wall is higher in anteroseptal OMI patients, than in normals.
Conculusions
We developed a useful method to evaluate regional wall function noninvasively by computer processed color images of WS on 2-D echocardiography.
