Left ventricular function in 133 patients with ischemic heart disease and 25 normal subjects was studied non-invasively with amyl nitrite inhalation, and the data were compared with the findings of left ventriculography and coronary angiography which was performed in 60 patients.
The ischemic heart disease group showed a significantly less increase of ET/PEP, left ventricular posterior wall excursion, mean posterior wall velocity, ejection fraction and mean Vcf and a significantly less decrease of LV diastolic dimension with amyl nitrite inhalation comparing to the control group, and the difference of the myocardial function reserve between the groups was made obvious by amyl nitrite inhalation. Among them, the change of ET/PEP was the most reliable method of comprehensive left ventricular myocardial function assessment, and it correlated well to the severity of the disease assessed by cardiac catheterizations.
With a good understanding of the meaning of each hemodynamic parameter, amyl nitrite provocation enables a comprehensive and precise non-invasive assessment of the left ventricular function in ischemic heart diseases.
