Myocardial Damage Detected by Two-Dimensional Speckle-Tracking Echocardiography in Patients with Extracardiac Sarcoidosis

Objectives: We aim to determine whether two-dimensional speckle-tracking echocardiography(2DSTE)can identify the myocardial damage detected by delayed enhancement magnetic resonance imaging(DEMRI)via the differences of myocardial deformation in extra-cardiac sarcoidosis patients who showed no structural and functional abnormalities in heart.
【Methods】
Forty-five patients with biopsy-proven extra-cardiac sarcoidosis were analyzed retrospectively. We excluded patients who have abnormal electrocardiographic and echocardiographic findings, including ventricular arrhythmias, heart block, or regional wall motion abnormalities, valvular heart disease, and cardiomyopathy. The 10 age-matched healthy control subjects were recruited as a control group. Comprehensive echocardiography and DEMRI were performed, and consecutively circumferential, longitudinal and radial strains were assessed by 2DSTE software in LV 16-segment model in accordance to presence(DE+)or absence(DE-)of DE.
【Results】
Of 45 patients, 36 segments in 13 patients showed DE. DE(+)segments had lower peak circumferential strain than DE(-)and control segments(-14±5%vs. -28±7%vs. -30±7%;P<0.0001). Peak longitudinal strain in segments with DE(+)was significantly decreased compared with the control segments(-19±4%vs. -23±5%;P = 0.005). However, peak radial strain was similar between 3 groups: 41±17%in DE(+)vs. 45±23%in DE(-)vs. 46±18%in control segments; P = 0.50.
【Conclusions】
Circumferential and longitudinal strains via 2DSTE can identify the myocardial damage detected by DEMRI in patients with extra-cardiac sarcoidosis.
