プライマリケアにおける負荷心エコー図

筆者は京都市で循環器科を標榜している開業医である.受診患者の平均年齢= 72歳と高齢の患者が多く,その9割が高血圧患者であり,1/4の患者が心房細動を合併している.
2000年に開業し,2001年からトレッドミル負荷心エコー図を試験的に開始した.2003年からストレスパッケージ搭載装置を用いてトレッドミル負荷心エコー図を実施している.同時にアデノシン3燐酸(ATP)負荷による冠動脈血流速予備能(CFVR)測定を開始した.
冠動脈病変の診断に関して,トレッドミル負荷心エコー図の感度87%,特異度80%,正診率84%であった.左前下行枝病変の診断に関して,CFVR測定の感度100%,特異度74%,正診率82%であった.しかし,冠動脈疾患の診断よりも,高齢高血圧患者に合併した拡張不全の診断に有用な場合の方が多い.労作時胸部症状を訴える高齢患者のうち,虚血所見を認めない患者の1/3で運動負荷後に僧帽弁流入血速度/弁輪部移動速度比(E/E’)= 15以上を認めた.運動負荷後にE/E’= 15以上を認める症例では,新規に心房細動を合併することが多い(ハザード比= 3.147).
このような,プライマリケアの現場における負荷心エコー図について述べようと思う.
