The patient was a 55-year-old woman who was admitted because of palpitation. Laboratory data on admission showed a thyroidal hyper function, a marked increase in concentration of plasma brain natriuretic peptide (BNP), and mild cardiomegaly on chest rodiogram. We diagnosed this clinical situation as primary hyperthyroidism. Thyroid function returned to normal and the palpitation ceased 1 month after treatment with thiamazole. Plasma BNP concentration decreased but remained slightly high for 4 months and returned to the normal range 11 months after admission. M-mode echocardiogram showed normal contraction of the left ventricular wall throughout the clinical course, and mitral inflow velocity by pulsed Doppler method showed the E>A pattern on admission and an abnormal relaxation pattern 4 months after admission. Cyclic variation of integrated backscatter (IB) value in the left ventricular posterior wall was small at time of admission (3.6 dB), had increased 4 months later (7.7 dB), and was close to the normal range 11 months toper admission (8.7 dB). We conclude that the IB ultrasound method is useful in evaluating myocardial damage in hyperthyroidism.
