Adoptive-immunotherapy (AIT) is a new therapeutic method which uses cultured self lymphocytes sensitized against self tumor antigens. We studied the changes in ultrasonographical (US) findings caused by AIT of hepatic metastasis of breast cancer in 19 patients.
Before AIT, most of their hepatic US images were those of typical metastatic liver cancer; multiple intrahepatic tumors with thick marginal hypoechoic zones (bull's eye pattern, 68%). In 3 patients (16%), tumors had disappeared on US images in 5 to 9 months after AIT (complete response, CR). In 9 patients (47%), tumors had decreased in their size, so that their borders and marginal hypoechoic zones had become unclear (partial response, PR), In the other 7 patients (37%) , tumors remained unchanged or rather increased in their size or revealed a diffuse metastatic pattern (non-response, NR). The effectivity of AIT was demonstrated in 63% judging from the US findings.
There was no tumor in which more hyperechoic change than that of surrounding tissue or cystic (necrotic) change was revealed to the US image. In some cases of CR, a diffuse unevenness or coarseness of the intrahepatic echo pattern remained after the disappearance of tumors. Follow up studies were considered to be nessesory in such cases because remaining of tumor cells at the microscopic level cannot be ruled out.
The US was found to be a sensitive as well as non-invasive method in detecting changes caused by AIT, so it is essential for any follow up studies.
