橋本病におけるエコーレベルおよびヒストグラムの有用性について

Ultrasonic Echo Level and Histogram in Hashimoto's Disease

Ultrasonic echo levels and histograms from 52 patients with diffuse goiter were examined for their associations with microsome test and serum T3, T4 and TSH levels. All of the patients were female and between 30 and 80 years of age.
1) Thirty-six of the 52 subjects had a positive microsome test. In these patients, the avarage echo level was 89.9, which was significantly lower than 98.9, the avarage found in the 16 microsome test-negative patients (p2) Twenty-eight (77.8%) of the 36 microsome test-positive patients showed a bell-shaped histogram with a narrow base which was characteristic of Hashimoto's disease.
3) Among the 39 patients with the Hashimoto's histogram pattern, those with a positive microsome test had an avarage echo level of 88.0, while those without had an average of 102.2. The differance was significant (p4) According to the serum levels of T3, T4 and TSH, the 28 patients with the Hashimoto's histogram pattern and a positive microsome test were further categorized into three subgroups: eu-; sub-eu-, and hypo-thyroidism. The avarage echo level corresponding to each of the three groups was 92.8, 86.8 and 78.9, respectively.
From these observarions, it was speculated that a combination of a bell-shaped histogram pattern with a narrow base and an low echo level may suggests Hashimoto's disease and that lowering of the echo level may, in part, reflect a deterioration of thyroid functions.
