超音波断層法による小児の脾臓計測—小中学生についての検討—

 We studied on the normal splenic size of school children and junior high school students (6-15 years old) with a real-time ultrasound scanner (TOSHIBA SAL-32B, 5 MHz linear array transducer). Splenotomogram was taken by the left intercostal scanning in the right lateral decubitus. Spleen Index (SI) could be measured in 172 (male: 94, female: 78) of 207 cases. SI was defined as "A×B " by Koga's method, in which A was the maximum line of the longitudinal section of spleen, and B was the perpendicular line to A through the splenic hilus. C was defined as the vertical thickness through the splenic hilus. D was the line between splenic hilus and splenic edge. E line was measured horizontally from splenic edge to the lung air echo of the upper posterior portion.
 SI was significantly correlated with the age, height, weight, and body surface area (BSA). (male: r=0.534, 0.572, 0.634, 0.621, female: r=0.504, 0.572, 0.627, 0.620: p Multiple regression analysis was performed on purpose to predict the SI of unmeasured cases. The prediction were presented underformula.
  Male: SI=8.21×C+2.43×E+0.149×Weight-15.7
  Female: SI=7.43×C+1.88×E+0.197×Weight-13.0
These models were well appropriate statistically but had problem to clinical application, because they had the factor of physique. And this analysis showed the probability of C as the index of one dimension that appeals splenic size of children.
